It is the policy of Able Hands Primary Care Center to maintain strict confidentiality and security of patients’ health information. Patient’s medical history will not be shared with any party except in the case of a valid search warrant or subpoena.
Patients have the right to request their medical records at any time. A release form must be signed for copies.
