What information should I bring along with me when I visit?
If this is your first visit to Able Hands Primary Care Center, please bring a government issued ID. If you are taking any medications, bring them in their original packaging, not just the loose pills. If you are taking insulin or any other medication that requires refrigeration, leave the medication in your refrigerator and bring the package only.
Include both prescription and over-the-counter medications. If you have results such as laboratory reports, ECGs, x-rays, ultrasounds, CT scans or MRI scans, bring them along. If you have been hospitalized or seen by another doctor and have a discharge summary or medical referral, please bring these as well, as they provide important information about your past and current health.
