Key Medical Services

Able Hands Primary Care Center provides primary care services for adults and children of all ages. Our services include both sick and well visits. These are included in the lists below:

Adult Services

Pediatric Services

The medical needs of children are very different from that of adults. Because children have smaller, more immature bodies, they respond differently to medications. Pediatric services include:

Our Commitment To Our Older Patients

As individuals age, their medical requirement become more complex and they require specialized care. At Able Hands Primary Care Center, we understand the unique needs of older patients and offer comprehensive geriatric medical care to address their health concerns.

In addition to providing specialized geriatric care, we work closely with family and caregivers to access community support to assist patients with obtaining medications through the NIB Drug Plan, medical supplies and disability window decal through the Department of Social Services. We have also supported admission to elderly daycare for older patients who benefit from this service. Able Hands Primary Care Center is committed to helping our patients maintain their independence and quality of life, and we work diligently to ensure that our older patients receive the individualized care they deserve.

SPECIALIZED FOCUS

Comprehensive Geriatric Services

Able Hands Primary Care Center provides comprehensie health care for older patients focusing mainly on functional capacity and management of chronic conditions. We focus on specialized care for complex, complicated geriatric syndromes. Additionally, when older adults can no longer care for, or make decisions for themselves, a legal guardian may be assigned to act in their best interest. We perform capacity assessments to help these patients and their families navigate the legal processes of securing a power of attorney or guardianship.

Our geriatric assessment protocol provides a comprehensive assessment of older patients who are experiencing memory loss or any other medical illnesses that make it difficult for them to perform important daily activities. We take a detailed medical history and perform a physical examination to assess our older patients. Also included as part of the patients evaluation is a review of current medications, a cognitive assessment test and social history investigation. We collaborate with family members and caregivers to develop a detailed plan that is best suited to promote the patient’s health and well-beign.
